Claim Eligibility
- You may request reimbursement if you've made out-of-pocket payments for services not covered by your health plan, visited a hospital not included in your plan, or exceeded your plan's coverage limit for certain services.
- Prior pre-authorization is required for non-emergency cases. Contact our Support Team to confirm authorization and reimbursement eligibility.
Required Documentation
To process your claim, you'll need to provide:
- Prior pre-authorization confirmation
- A medical report (in some cases)
- Receipt of payment
Claim Submission
- Inform Bastion Health within 72 hours of the payment and submit your claim within 1 week of receiving care.
- Access the reimbursement form via our mobile app's "We Care" section. Attach your payment receipt and submit the form for processing. Please see this section for detailed instructions on requesting reimbursements via the mobile app.
Processing Time
- We typically process reimbursements within 1 month of receiving requests, ensuring timely resolution for our valued clients.
